All activities are delivered by
3 Minds QLD and are billed in line with the current
NDIS Price Guide.
� Location:
All programs start and finish at:
3 Minds QLD – Shop 4a, 73–77 George Street, Beenleigh
� Hours of Support:
Standard program hours are
9:00 AM to 3:00 PM, unless otherwise specified in the activity details.
Extended hours may be available on request and will be billed accordingly.
� Price Structure:
1:1 Support: $67.56 per hour
1:2 Support: $33.78 per hour
Travel: $1.00 per kilometre
Travel for group outings is split between participants
Additional transport (e.g. pick-up/drop-off) is charged per km, either at full rate for 1:1 or divided among those sharing transport
� Pick-Up & Drop-Off:
Available upon request at an additional cost. Please advise at time of booking.
� Out-of-Pocket Expenses:
Some activities include a set fee for participation (e.g. tickets, snacks, materials). This amount is noted in the activity details and is
not claimable under NDIS funding.
Any extra items participants may choose to buy (e.g. souvenirs, café food, personal purchases) are the responsibility of the
participant or guardian.
Payment of all out-of-pocket expenses is required prior to attending and can be transferred directly to
3 Minds QLD.
